How do I change my name?

Submit a copy of your updated driver's license or social security card through our secure document upload form below. Please be sure to include the name currently listed on your account and what it needs to be changed to, your PEBP ID and the last four of your SSN to ensure the document is associated with the correct account.

How do I change my contact information (address, email, phone number) with PEBP?

 

You can update your contact information including your address, phone number and email address by logging into your E-PEBP portal and submitting a question. Additionally, you may call Member Services at 775-684-7000, 702-486-3100 or 1-800-326-5496 and select option 2 to have your contact information updated over the phone.

How do I submit supporting documentation?

You can upload documents in your E-PEBP portal for an accompanying event or attach the document(s) to our secure document upload form on our Contact Us page under Supporting Documents.

I am a CDHP, LD, or EPO participant, how can I order a replacement ID card?

  1. Login to the E-PEBP Portal
  2. Select Click here to access ‘UMR’ under Quick Links on the left-hand side of the page
  3. Once on the UMR website, click on “ID Card” under the myMenu tab on the left side if the page. Here you can view, order, or fax your ID card to a provider.

I am a CDHP, LD, or EPO participant, how do I view my claims?

  1. Login to the E-PEBP Portal
  2. Select Click here to access ‘UMR’ under Quick Links on the left-hand side of the page
  3. From the UMR home page, click on "Claims" tab under myMenu to view current and past medical and dental claims

How do I establish an online account?

  1. Login to the E-PEBP Portal
  2. Click on Create an account to set up a new E-PEBP Portal account
  3. Provide your identifying information and follow the prompts to set up your account
  4. Select and provide answers to your security questions
  5. Set your password; your password must be between 8-15 characters, contain at least one number, and have at least one uppercase and one lowercase letter

How can I change my HSA contribution mid-year?

Once your beneficiary designation has been completed in your E-PEBP portal you will be able to complete the Employee HSA Contribution Change event. If you try to complete this event without beneficiaries on file, the event will not process.

Instructions to access the Change Beneficiary Designation event:

  1. Log into your E-PEBP portal
  2. Upon entering your credentials, you will land on the homepage
  3. From the homepage, select the Quick Actions link on the Medical Widget/Benefit Card
  4. Then select Enroll or Make Changes
  5. Under Anytime Change, select Change Beneficiary Designation
  6. Enrollment is complete when you see “Enrollment Confirmed”

Instructions to access the Employee HSA Contribution Change event:

  1. Log into your E-PEBP portal
  2. Upon entering your credentials, you will land on the homepage
  3. From the homepage, select the Quick Actions link on the Medical Widget/Benefit Card
  4. Then select Enroll or Make Changes
  5. Under Anytime Change, select EE HSA Contribution Change
  6. Enrollment is complete when you see “Enrollment Confirmed”

 

Adjustments made any time during the month will be applied to the payroll deduction the following month. Funds will show in member’s HSA Bank account an average of 72 hours, or longer depending on weekends and holidays, after the payroll date in which it was deducted.

You must still update your beneficiaries with HSA Bank using HSA Bank’s HSA Designation of Beneficiary Form. Please complete, sign, and mail/email this form to: HSA Bank, PO Box 939, Sheboygan, WI 53082; or hsaforms@hsabank.com. Your beneficiary designation will not be complete until HSA Bank receives your completed and signed form.

I am a new employee, how do I enroll in a medical plan?

  1. Log in to the E-PEPB Portal.
  2. Create an account or login to your existing account. 
  3. The system will guide you through the steps to elect a medical plan and add your dependents (if applicable).

Once you have completed the online process, you will need to submit any required supporting documents to the PEBP office. Applicable supporting documentation can be uploaded to your portal once you complete your enrollment. If applicable: Submit a copy of the certified marriage certificate, domestic partner certification, and/or certified birth certificate(s). Social Security number(s) for spouse/domestic partner and/or covered children must be provided.

I want to change my medical plan, how can I do that?

You may change your medical plan option during the open enrollment period, generally May 1st through May 31st. Changes made during open enrollment become effective July 1st. However, if you are enrolled in the EPO or HMO plan and move outside of your coverage area, you are eligible for a special enrollment period and can switch to the Consumer Driven Health Plan or Low Deductible Plan to continue PEBP coverage. There are also exceptions for Qualified Medical Child Support Orders (QMCSO).

I am newly married and want to be covered under my spouse's plan, how do I decline PEBP's coverage?

Within 60 days from the date of the event, submit a copy of your certified marriage certificate and proof of new employer group health insurance to our secure document upload form on our Contact Us page under Supporting Documents. Be sure to communicate that you want to decline your coverage in the optional message area of the form.

How do I participate in the Diabetes Care Management or Obesity Care Management Programs?
