Basic Life Insurance

UnitedHealthcare is the plan administrator for the basic life insurance policy provided to eligible active and retired members effective 7/1/2022.

For basic life insurance death claims where the participant was deceased prior to 7/1/2022 contact The Standard—Customer Service: 1-888-288-1270.

To be eligible for the basic life insurance, you must be covered under the PEBP sponsored medical plan, and be in one of the following classes:

  • Class 1: Full-time employees of the State of Nevada (or any non-State agency approved by the PEBP board), professional full-time employees of the Nevada System of Higher Education (under annual contract), and members of the Nevada Senate or Assembly are all eligible for this benefit. A full-time employee is one who works at least 80 hours per month. Your employer pays the full cost of basic life insurance.
  • Class 2: retirees of the State of Nevada receiving PERS, or judge retirement benefits and legislators qualifying under Chapter 242 of the Sessions Law of the sixty-third Session of the Nevada State Legislature (or NRS 287.045), professional employees qualifying per NAC 287.135, and retirees eligible to join PEBP upon retirement pursuant to NRS 287.023 are eligible for this benefit. Reinstated retirees are not eligible for basic life insurance benefits or voluntary life insurance coverage. Certain retirees pay a contribution toward the cost of basic life insurance.

State Actives/Retirees are those whose last employer is a State agency, NSHE, PERS, the Legislature, Legislative Counsel Bureau or a State Board or Commission. Non-State Actives/Retirees are those whose last employer is a non-State public entity (a local government that is contracted with PEBP to provide coverage to their active employees pursuant to NRS 287.025). Non-State retirees are eligible to join PEBP only if their last employer is a participating local government entity (a local government that is contracted with PEBP to provide coverage to their active employees pursuant to NRS 287.025). If the participating local government entity leaves the PEBP Plan, the entity’s retirees will also be dis-enrolled unless the retiree was covered under PEBP as a retiree continually since November 30, 2008. Retirees who were covered under PEBP as a retiree on November 30, 2008, and continually since then may remain covered under PEBP if they continue to pay their premiums.

Basic Life Insurance Forms and Resources

Basic Life Insurance Claim Submission
When an active employee or retiree who is covered by a PEBP Plan passes away, they may be entitled to a basic life insurance benefit. To initiate the life insurance claim process, please complete the “Claimant’s Statement” on page one of the “Life Claim Form.” You may also wish to complete the “Claims Department Direct Deposit Agreement For Payment of Benefit to Financial Institution” section on the last page of the claim form if you wish to receive any applicable benefit by direct deposit. Once you have completed the required claim form, please submit it to the PEBP office along with a copy of the certified death certificate (you do not need to send the original). Once the claim has been received, PEBP will complete the “Employer’s Statement” on page two of the claim form to determine eligibility and submit the form along with the death certificate to PEBP’s basic life insurance administrator, UHC, for processing. If an additional voluntary life insurance benefit is identified, PEBP will submit the required form and documentation to our voluntary life insurance administrator The Standard. All life insurance claims require review to ensure eligibility in compliance with the PEBP master plan documents.

Please note: Adjustments in premiums resulting from the death of a covered participant or dependent will be refunded if notification of death is received within 60 days of the participant’s or dependent’s date of death. Notification of death beyond the 60-day period will not be refunded.

Survivor Benefits
Eligible surviving dependent(s) who wish to continue coverage under PEBP must complete an enrollment election by submitting the required form to PEBP within 60 days of the date of death of the employee or retiree. Surviving dependents include a participant’s spouse or domestic partner and dependent children to age 26 years (or to age 19 years for child(ren) under permanent legal guardianship) who are covered under the participant’s medical Plan on the date of the participant’s death. If the decedent was an active employee (not a retiree), they must have at least 10 years of services at time of death for the surviving dependents to be eligible. To apply for survivor benefits, please complete the Retiree Benefit Enrollment and Change Form (RBECF) at https://pebp.nv.gov/Resources/forms/forms/ and select the option for “Survivor Election”. Rates for survivor benefits are unsubsidized and enrollees are responsible to pay for the full cost of premiums for their coverage and insurance to PEBP. Survivor benefits will initiate on the first day of the month following the participant's death dependent on submission of required forms and eligibility requirements.

The Member Assistance Program (MAP)

The UnitedHealthcare Member Assistance Program (MAP) is included as part of the basic life insurance policy for eligible members enrolled in the Consumer Driven Health Plan (PPO), Low Deductible Plan (PPO), Exclusive Provider Organization Plan (EPO), and the Health Plan of Nevada (HMO). Travel Assistance

The UnitedHealthcare Global Travel Assistance Program is included as part of the basic life insurance policy for eligible members enrolled in the Consumer Driven Health Plan (PPO), Low Deductible Plan (PPO), Exclusive Provider Organization Plan (EPO), and the Health Plan of Nevada (HMO). This benefit is for you or your eligible dependents when traveling 100 miles or more away from home or outside the country.
